The surveys created by Inspira Health asks, “Do you identify your baby as a transgender male/ trans man/ female to male”, with additional options for “genderqueer” and an “additional gender category” according to The Telegraph.

InfoWars reports: The surveys also asked to know whether babies were “lesbian or gay,” “heterosexual,” “self-describes” or is “questioning/unsure.”
Health care provider Inspira Health said the survey was required by New Jersey law, but that parents were “permitted to decline to provide” the data.
The Inspira Health network, which operates about a dozen medical facilities throughout the state, told The Telegraph New Jersey law requires them to collect info on “race, ethnicity, sexual orientation, and gender identity in a culturally competent and sensitive matter.”
Inspira claims they sought a waiver from the state on the survey, but it has yet to be granted.
Parents speaking to the NY Post were stunned by the survey, with New Jersey based mother-of-two Sandy Anello saying, “That form is completely crazy, and anyone who would dictate a baby’s sexual orientation probably has an agenda.”
“If I was told to fill this out, I’d rip it up in front of them. It feels like we’ve entered ‘The Twilight Zone,’” she added.
Another soon-to-be mother commented, “Identifying my baby as gay on Day 1 is insane,” adding “I had no idea about this form and I’m shocked.”
New Jersey State Senator Holly Schepisi (R) also slammed the tone-deaf survey being distributed to mothers recovering from child-birth.
“The entire thing lacks such common sense, and serves no purpose whatsoever,” Schipisi said.
“As a mom myself, I know you’re exhausted after giving birth, you’ve got a crying newborn and you’re trying to figure out how to feed it. To be handed that sort of form in the midst of all that has no medical value, it makes no sense.”
While the questionnaire is stirring outrage, footage last year showed some delusional American parents actually believe children as young as one are able to choose their gender, illustrating just how deeply the leftist transgender indoctrination agenda has infiltrated the culture.
